### <mark style="background-color:$danger;">文件提供注意</mark>

**請提供的文件部分需要提供最新版本**

* 最新版本負責人身分證
* 負責人須年滿18歲

#### 1. 如果有「證件指定特定用途」加上浮水印之需求(害怕證件遭到盜用)

務必以下方二<mark style="color:$danger;">**擇一**</mark>填寫：

① 僅供申請LINE Pay / LINE Pay Money 使用。

② 僅供連加網路商業股份有限公司/ 連加電子支付股份有限公司 使用。

#### 2. 存摺部分需要留意以防申請退件

1. 該銀行戶頭與申請統編不一致 (總分公司，請與申辦銀行確認，該存摺為何統編做申辦)
2. 該銀行戶頭於銀行端已停用
3. 存摺為籌備處 (請與申辦銀行確認，是否需更換為正式存摺)
4. 存摺為外匯存摺 (無法使用外匯存摺，請更換其他存摺)
5. 該統編曾申辦過 (同一負責人原帳號存摺與現申辦不一樣，個人存摺與公司存摺只能擇一使用)
6. 該統編曾申辦過 (前負責人曾申辦過)
7. 該存摺為專戶、支存戶、外商銀行、台中銀行、永豐銀行、王道銀行、虛擬帳戶、信託帳戶

#### 3. 實體line pay需多提供

1. 店面與商品佐證資料
2. 店外招牌及店內照片
3. 含價格商品菜單或價目表或標價商品圖

商城或是粉專都<mark style="color:$danger;background-color:yellow;">不可</mark>出現的字眼如下:

<mark style="color:$danger;">儲值金</mark>-屬**遞延性商品**「先付費購買，過一段時間後才享受服務」需提供履約保證相關文件完整內容。 \
如有出現要簽屬履約保證
